Community Health Alliance Earns National Recognition for Quality Care and Innovation

These badges highlight the dedication and hard work of over 300 employees across their six fixed-site locations and administrative office, all of whom are committed to improving the health and well-being of our community.

Community Health Alliance (CHA) is proud to announce its recent recognition by the Health Resources and Services Administration (HRSA) and the National Committee for Quality Assurance (NCQA), having received multiple Community Health Quality Recognition badges. These badges highlight the dedication and hard work of over 300 employees across their six fixed-site locations and administrative office, all of whom are committed to improving the health and well-being of our community.

Among the honors, CHA received the HRSA Health Center Quality Leader (Bronze) badge, placing them among the top 30 percent of health centers nationwide for quality of care. This recognition is a testament to the exceptional service and care their entire team provides to their community every day.

CHA also received the HRSA Access Enhancer badge, a recognition of their ongoing efforts to broaden access to healthcare services. This badge highlights their unwavering commitment to ensuring that every member of their community has the opportunity to receive essential care, regardless of their circumstances. A large part of their efforts have been expanding their health centers, along with adding new services lines. In addition, over the last 18 months, CHA has hired 33 new providers across various disciplines, including medical, dental, behavioral health, and pharmacy.

CHA was also recognized with the HRSA Addressing Social Risk Factors badge, reflecting their focus on tackling the social determinants of health that affect their patients. By addressing these critical factors through their newly established Community Health Worker Program, they are able to provide more comprehensive and effective care to those who need it most.

The HRSA Advancing Health Information Technology (HIT) for Quality badge further emphasizes CHA’s leadership in leveraging innovative technology to improve quality of care. By integrating the various health IT solutions, such as their Patient Portal and the recent launch of an online Symptom Checker, they have enhanced patient outcomes and optimized their operations.

In addition, CHA was honored with the HRSA Health Disparities Reducer badge, recognizing their successful efforts in reducing health disparities within their patient population. This achievement speaks to their dedication to delivering equitable care to all, regardless of background or circumstances.

Finally, Community Health Alliance is honored to have received the prestigious Patient-Centered Medical Home (PCMH) recognition from NCQA, a testament to their commitment to delivering high-quality, comprehensive, and coordinated care. This designation reflects their dedication to putting patients at the heart of everything they do, fostering stronger patient-provider relationships and improving health outcomes. By embracing the PCMH model, they continue to focus on personalized care, teamwork, and accessibility to ensure the well-being of their community.
