Community Health Alliance receives Title X federal funding, ensuring every person access to basic, preventative reproductive health care. This includes wellness exams, cervical and breast cancer screenings, birth control, contraception education, testing and treatment for sexually transmitted infections (STIs), as well as HIV testing. We can also assist you in family planning and birth spacing.
For women who are underinsured or uninsured, we offer the Women’s Health Connection program. This program helps women from ages 21-64 cover their annual physical including screening exams such as pap smears, mammograms (diagnostic mammogram imaging) and colposcopies. If you fit into this category, please fill out a Women’s Health Connection form and drop it off at your nearest Community Health Alliance health center. After dropping off this form, our Cancer Care Coordinator will contact you to schedule your annual physical.
Women’s Health Connection forms are categorized by age and can be downloaded and printed from home:
- If you are between the ages of 21-39, please fill out this form: English | Spanish
- If you are between the ages of 40-64, please fill out this form: English | Spanish
